Privacy Statement

At SRS, we believe your personal information is taonga, and we work hard to maintain the privacy, security and integrity of any information held about you. We abide by the principles and requirements set out in the Privacy Act 2020 and the Health Information Privacy Code 2020.

Below is some information about the information we collect and how we use it, how we store it and who we share it with, and how you can view, correct, change or delete your personal information.

How we collect personal information

We may collect information from you in a variety of ways including face-to-face, over the telephone or using telehealth, or using paper forms, email or online forms. We also are provided information from other sources when we receive a referral for you. Sometimes, with your consent, we may need to collect information about you from another source. We may also need to collect information from another source if we are unable to collect the information we need from you.

How we use personal information

We use personal information to provide services to you such as assessment and rehabilitation. We may also collect personal information that is necessary for us to meet legal or regulatory obligations.
Providing some information to us is optional. If you choose not to provide information, we may be unable to provide you with our services.

Who we share personal information with

We will only share personal information if we are using it for the reason we collected it, if you have given your consent, or where there are other legal or compliance reasons (for example, if this is necessary to prevent harm).

How we store personal information

We keep your information safe by storing both electronic and paper-based information securely, following New Zealand government standards for the storage of personal information.
We keep your information for ten years from our last contact with you, at which point we securely destroy it by securely erasing digital and destroying paper-based information.

How you can view, correct, change or delete your personal information

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. We will consider all requests and will be able to make changes to information that is deemed to be factually incorrect, or you can provide a written statement of correction, which we will add to your file as an addendum and will show that you requested a change to your information, but that we haven’t made the change.
